For individuals who are transitioning from therapeutic to Para Sport, please complete the following forms:

• PEA Obrien Liability Release
• PEA Medical Release
• PEA Participant Application
• Photo Release Form

*​Physician Release is necessary to ensure participant’s safety in all mounted activities involving equine assisted participation.
